Though you don't need a referral to visit Bone & Joint, sometimes a provider will refer a patient to us. Our physician referral area makes the referral process easier.

Four items you should bring to your first appointment.

Please arrive early so we can complete your initial paperwork. Also bring the following:

  1. Insurance card
  2. A list of current prescriptions, over-the-counter medications and/or supplements. Including dose and frequency
  3. Medical and surgical history
  4. Recent test results, X-rays or other relevant records
